2025 OKM Junior Tennis Registration

Welcome to the 2025 OKM Junior Tennis registration!

  • Please make sure you finish all of the sections and get a confirmation email to show that you finished the registration.
  • You must have an OKM Club Family Membership that is paid for 2025 before signing up for OKM Junior Teams.
  • This is a consolidated registration form for both Full/Bonded members AND Associate members (which is different than the Swim/Dive Form).  You'll mark on the form which membership type you have.  There's a $25 price discount for Full/Bonded athlete registrations, and you'll make that selection on the 2nd Tab called "Athlete Groups".
  • Despite separate forms, we are still doing a multi-registration discount of $25 off for the 3rd family registration and beyond (across all OKM Junior Teams).  See info on discount codes in the payment section of this form.  If you are doing both tennis and aquatic sports, we ask that you apply the discount code only on the Swim/Dive Form.  If you are doing tennis only and have 3+ participants, you'll apply the discount code on this Tennis form. 
  • We are continuing the same practice schedule this year, and you'll need select a specific practice session you are choosing in the Merchandise Section of this registration form.  Each class session will be capped at 24 participants and will close after it's full.  Here's the practice schedule once school is out: 
    • #1 - 8:00-8:45AM (ages 5-10)   2 of 24 sports are filled  
    • #2 - 8:45-9:30AM (ages 5-10)    ****FULL****    
    • #3 - 9:30-10:30AM (ages 11+)   ****FULL****    
    • #4 - 10:30-11:15AM (ages 9-12)  17 of 24 spots are filled    
    • #5 - 11:15AM-12:00PM (ages 5-10)  23 of 24 spots are filled
    • #6 - 12:30-1:30PM (ages 11+)    2 of 24 spots are filled         
  • If athletes are doing multiple OKM sports, please make sure you know their practice schedule for other sports and choose your tennis classes accordingly.

Athletic Waiver & Release of Liability

As Parent/Guardian of the above-named minor(s), I grant permission for them to participate in all activities of the OKM Junior Teams, which are Northern Virginia Swim League (NVSL) or Northern Virginia Tennis League (NVTL) member teams. I represent and warrant that my minor child/children participating on the OKM Junior Teams are in good health and have no physical condition, ailment or disability which renders them unable to participate in vigorous physical activity.

For and in consideration of benefits derived from participation in the OKM Junior Teams, I understand that: (a) the risk of injury to my child/children from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and (b) there is a risk of possible exposure to and illness from infectious and/or communicable diseases, including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist.

I assume all risks and hazards arising out of, or related to, such participation, including, but not limited to, transportation to and from such activities, and do hereby indemnify, release and hold harmless OKM, its officers, coaches, participants, team representatives, volunteers, employees and agents, as well as the NVSL and NVTL, from all claims of any kind whatsoever which may arise or hereafter accrue in connection with my child's/children's participation in activities of the OKM Junior Teams. I further grant permission for first aid or medical treatment to be given to my child/children in an emergency and will be solely responsible for any medical costs which may arise.

OKM Junior Teams Standards of Conduct

These Standards of Conduct are intended to promote respect, fairness, civility, honesty, responsibility, and appropriate behavior by all OKM athletes and parents. Inappropriate, abusive, profane or violent behavior or language by an athlete or parent in connection with any OKM Junior Team may result in either Member suspension or expulsion, without refund, from OKM Junior Sports. Member suspension or expulsion shall be decided by the OKM Board after consultation with the appropriate coaches and team representatives.

All Athletes, Coaches, Team Representatives, Officials, Parents I Spectators, shall:

  • Abide by the NVSL and NVTL Codes of Conduct.
  • Abide by the established rules of their OKM Junior Sports Team.
  • Respect all officials at all times, and address them in a courteous manner.
  • Avoid harassment, aggressive shouting, using inappropriate language, profanity, humiliating or intimidating remarks or behavior, verbal or physical abuse toward any athlete, coach, official, or spectator.
  • Not engage in intimidating, disrespectful or negative behavior toward officials, athletes, other spectators, support personnel, or staff.
  • Follow applicable management, pool, and facility rules, respecting others' property at all times.
  • Display respect, courtesy, and good manners toward athletes, coaches, Team Representatives, and spectators.
  • Demonstrate humility in victory and courtesy in defeat.
